【摘要】 目的 分析血清单核细胞趋化蛋白-1 (MCP-1)、白细胞介素-35 (IL-35)、热休克蛋白-70 (HSP-70) 对消化性溃疡并发出血的预测价值。方法 选取 2021 年 10 月至 2023 年 10 月开封市第三人民医院收治的 300 例消化性溃疡患者作为研究对象, 根据是否并发出血分为出血组 (125 例) 与未出血组 (175 例), 收集患者一般资料、疾病相关情况以及血清 MCP-1、IL-35、HSP-70 水平等资料, 分析血清 MCP-1、IL-35、HSP-70 水平对消化性溃疡并发出血的预测价值。结果 多因素 Logistic 回归分析结果显示, 有非甾体类抗炎药 (NSAIDs) 应用史及幽门螺杆菌 (Hp) 感染、溃疡长径较长以及血清 MCP-1 与 HSP-70 水平相对较高、IL-35 水平相对较低是消化性溃疡并发出血的独立危险因素 (95%CI 为 1.088 ~ 3.027、1.186 ~ 6.058、1.037 ~ 1.138、1.895 ~ 16.889、0.082 ~ 0.498、1.309~ 11.347, P= 0.022、0.018、0.001、0.001、0.001、0.014); 受试者工作特征 (ROC) 曲线分析结果显示, 联合检测血清 MCP-1、IL-35、HSP-70 水平预测消化性溃疡并发出血的敏感度为 94.5%、特异度为95.0%。 结论 联合检测血清 MCP-1、IL-35、HSP-70 水平预测消化性溃疡并发出血的价值较高。 |

【Abstract】 Objective To analyze the predictive value of serum monocyte chemoattractant protein-1 (MCP-1), interleukin-35 ( IL-35), and heat shock protein-70 ( HSP-70) levels for peptic ulcer complicated with hemorrhage. Methods 300 patients with peptic ulcer admitted to Kaifeng Third People’s Hospital from October 2021 to October 2023 were selected as research subjects and divided into the hemorrhage group ( n = 125) and the non-hemorrhage group ( n = 175) based on the occurrence of hemorrhage. Their data were collected, including general data, disease relevant status, and serum levels of MCP-1, IL-35, and HSP-70. The predictive value of serum MCP-1, IL-35, and HSP-70 levels for peptic ulcer complicated with hemorrhage was analyzed. Results Multivariate Logistic regression analysis showed that history of application of non-steroidal anti-inflammatory drugs (NSAIDs) and Helicobacter pylori (Hp) infection, longer ulcer diameter, and higher serum MCP-1 and HSP-70 levels, lower IL-35 levels were independent risk factors for peptic ulcer complicated with hemorrhage (95%CI: 1.088-3.027, 1.186-6.058, 1.037-1.138, 1.895-16.889, 0.082-0.498, 1.309-11.347,P= 0.022, 0.018, 0.001, 0.001, 0.001 and 0.014). The receiver operating characteristic (ROC) curve analysis showed that the sensitivity of the combined detection of serum MCP-1, IL-35, and HSP-70 levels in predicting peptic ulcer complicated with hemorrhage was 94.5%, and the specificity was 95.0%. Conclusion The combined detection of serum MCP-1, IL-35, and HSP-70 levels presents a high value in predicting peptic ulcer complicated with hemorrhage. |
