          【摘要】 目的 分析总结湿润烧伤膏与自体微粒皮种植在高龄多发Ⅳ期压疮患者中的应用效果。方法 选取 2018 年 6 月至 2022 年 12 月北京市丰台区中医医院收治的 57 例高龄多发Ⅳ期压疮 (创面 285 处) 患者为研究 对象, 给予其在抗感染、调控血糖及血压、改善微循环、营养支持等综合治疗的基础上, 应用湿润烧伤膏治疗局部创面, 并给予条件允许者于新鲜肉芽组织生长良好后行自体微粒皮种植, 观察记录患者创面愈合情况、死亡率及其生存时间。 结果 57 例患者中共 37 例 (创面 228 处) 于创面肉芽组织生长良好时进行了自体微粒皮种植, 且种植成功率为 96.49% (220 / 228)。 另外, 57 例患者中共 25 例 ( 43.86%) 创面愈合出院, 愈合时间为 40 ~ 259 d (平均 98.52 d); 27 例 (47.37%) 因临终状态 (6 例, 死亡时创面无明显变化) 或其他合并症 (21 例, 死亡时创面明显缩小) 死亡, 住院生存时间 1~ 437 d (平均 81.15 d); 5 例 (8.77%) 中途放弃治疗, 自行出院。结论 湿润烧伤膏或联合自体微粒皮种植治疗高龄多发Ⅳ期压疮患者创面, 能够有效减轻患者痛苦, 促进创面愈合, 延长患者生存时间。

          【Abstract】 Objective To analyze the clinical efficacy of moist exposed burn ointment (MEBO) combined with autologous microskin grafting in elderly patients with multiple stage Ⅳ pressure ulcers. Methods 57 elderly patients with multiple stage Ⅳ pressure ulcers (285 wounds) admitted to Beijing Fengtai Hospital of Traditional Chinese Medicine from June 2018 to December 2022 were treated with MEBO for local wound care on the basis of systemic treatments such as anti?infection, blood glucose and blood pressure control, microcirculation improvement, and nutrition support. Autologous microskin grafting was performed for patients with well?grown granulation tissue. Wound healing, mortality rate, and survival time of patients were observed and recorded. Results Among the 57 patients, 37 patients (228 wounds) underwent autologous microskin grafting when the wound granulation tissue grew well, achieving a success rate of 96.49% (220 / 228). In addition, 25 (43.86%) of the 57 patients healed and were discharged, with the healing time ranging from 40 to 259 days (average: 98.52 days). 27 patients (47.37%) died due to terminal conditions (6 cases, with no obvious changes in the wounds at the time of death) or other complications (21 cases, with significantly reduced wounds at the time of death), with the hospital survival time ranging from 1 to 437 days (average: 81.15 days). 5 patients (8.77%) gave up treatment halfway and were discharged voluntarily. Conclusion The application of MEBO combined with autologous microskin grafting in treating elderly patients with multiple stage Ⅳ pressure ulcers can effectively relieve the pain of patients, promote wound healing, and extend the survival time of patients.
