          【摘要】目的 探讨超脉冲CO2点阵激光联合湿润烧伤膏治疗创伤性瘢痕的临床疗效。方法 选取2016年3月至2019年1月驻马店市中心医院整形美容科收治的62例创伤性瘢痕患者作为研究对象,并按照随机数表法将其随机分为观察组与对照组,每组31例。观察组患者采用超脉冲CO2点阵激光联合湿润烧伤膏治疗,对照组患者采用超脉冲CO2点阵激光联合糠酸莫米松乳膏治疗,对比观察两组患者温哥华瘢痕量表(Vancouver scar scale,VSS)评分,临床疗效及不良反应发生情况。结果 治疗后,观察组患者VSS评分为(4.43±2.01)分,明显低于对照组患者的VSS评分(6.57±1.94)分,差异具有统计学意义(t=4.265,P=0.000);观察组患者的总有效率为90.32%,明显高于对照组患者的总有效率74.19%,差异具有统计学意义(Z=-2.181,P=0.029);观察组患者的不良反应发生率为16.13%,明显少于对照组患者的不良反应发生率38.71%,差异具有统计学意义(χ2=3.971,P=0.046)。结论 超脉冲CO2点阵激光联合湿润烧伤膏治疗创伤性瘢痕,可有效改善瘢痕色泽和质地,提高治疗有效率,且不良反应较少,安全性较高,对临床具有重要意义。

          【Abstract】 Objective To explore the clinical effect of super-pulse fractional CO2 laser combined with MEBO in the treatment of traumatic scars. Methods 62 patients with traumatic scars, admitted to the Department of Plastic Surgery and Cosmetology of Zhumadian Central Hospital from March 2016 to January 2019, were selected as subjects, and then divided, according to the random number table, into an observation group (31 cases) and a control group (31cases). Patients in the observation group were treated with ultra-pulsed fractional CO2 laser combined with MEBO, while patients in the control group were treated with ultra-pulsed fractional CO2 laser in combination with mometasone furoate cream. The Vancouver scar scale (VSS) score, clinical effect and adverse reactions were compared between the two groups. Results After the treatment, the VSS score of the observation group was (4.43 ± 2.01) points, which was significantly lower than the control group (6.57 ± 1.94 points) and the comparison showed statistically significant difference (t = 4.265, P = 0.000); the total effective rate of the observation group was 90.32%, which was significantly higher than the control group (74.19%), and the comparison showed statistically significant difference (Z = -2.181, P = 0.029); the adverse reaction rate of the observation group was 16.13%, significantly lower than the control group (38.71%), and the comparison showed statistically significant difference (χ2 = 3.971, P = 0.046). Conclusion Ultra-pulsed fractional CO2 laser combined with MEBO in the treatment of traumatic scars can improve the color and texture of scars and increase treatment efficiency, and it has little adverse reaction and high safety, so it is of great significance to the clinical practice.
