          【摘要】 增生性瘢痕具有发病率高、治疗难度大、易影响局部功能及易复发等特点,临床治疗较为棘手。近年来,随着体外冲击波疗法(extracorporeal shock wave therapy,ESWT) 的不断普及,部分研究学者发现,其能够在无创条件下到达瘢痕组织深层,对紊乱的胶原组织进行松解,从而改善瘢痕外观、缓解瘢痕疼痛及瘙痒等症状,在增生性瘢痕的治疗中具有独特的优势。然而,目前该项研究尚处于初始阶段,关节处增生性瘢痕应用ESWT 治疗的研究以及ESWT 治疗增生性瘢痕作用机制的研究均较少,且治疗参数、频率、剂量等的设置问题尚缺乏统一标准。为此,笔者对ESWT 治疗增生性瘢痕的研究进展进行了综述,并分析了其治疗增生性瘢痕的作用机制,旨在为ESWT 治疗增生性瘢痕的进一步研究提供帮助。

          【Abstract】 Characterized by high incidence and relapse, and high probability of impairing local functions of body, hyperplastic scar is difficult to treat and remains a thorny problem in clinical practice. In recent years, extracorporeal shock wave therapy (ESWT) has gained increasing popularity and some researchers have found that ESWT can reach the deep layer of scar tissues non-invasively to loose disordered collagen tissues, thereby improving the appearance of scar, alleviating scar pain and itching, presenting unique advantages in the treatment of hyperplastic scar. However, such research is still at its initial stage, and few studies have been carried out on the application of ESWT in hyperplastic scars at joints and the action mechanism of ESWT in the treatment of hyperplastic scars, and moreover, there is no uniform standard yet for some settings such as treatment parameters, frequency, dosage and etc. To this end, this paper made a review on the research progress of ESWT in the treatment of hyperplastic scars and analyzed its mechanism of action on this aspect, with the purpose of providing reference for the further research on ESWT in treating hyperplastic scars.
